The Shrieking Shack is completely dilapidated and vandalized on the inside, although the film never explicitly explains why, nor how Sirius and Lupin know how to enter it via the tunnel at the base of the Whomping Willow. The book explains that Professor Dumbledore allowed young Lupin to study at Hogwarts, on the condition that he would isolate himself in the purposely built shack every month during his werewolf transformations, so that he couldn't hurt anyone. The Willow was planted over the tunnel entrance to keep other students from entering, and the monthly sounds of Lupin howling and thrashing the interior quickly established the reputation that the shack was haunted. In fact, Dumbledore encouraged these stories so that people wouldn't go near it. When Lupin's friends James, Sirius and Peter learned of his secret, they became Animagi so that they could join him in the shack each month (since werewolves are no danger to animals). In the book, Harry also learns the nature of Snape's hatred towards James and Sirius: when Snape found out that Lupin headed into the shack each month, Sirius jokingly encouraged Snape to follow Lupin to learn his secret. Realizing that this could kill Snape, James was barely able to save Snape from being attacked by Lupin in his werewolf form. Snape always held a grudge towards Sirius and James, believing that they had tried to lure him to his death but changed their minds, not out of fear for Snape's well-being, but of getting expelled from school.
