While there appears to be reduced harm for Il-Nam at some points throughout the games, during tug-of-war, you can see he was fully locked in with the rest of his team for the entirety of the round. He already has an imminent, terminal cancer diagnosis and openly stated he played the games because he was bored - based on this alone, it seems unlikely he would take measures to cheat and save his life as it would take the excitement out of the experience and all he wanted was to have fun.
