In the forged documents that Bella receives from J. Jenks, Jacob's last name is given as "Wolfe"- a clear play on his supernatural status. In the books, Bella is the one who gives the name, because she thinks Jacob will find it funny. During the scene, J. Jenks states that "Mr. Jasper" put in the order and Bella is just picking it up. So, it can be assumed that either Alice or Jasper chose the last name Wolfe.
