The original ending of "An Unlocked Window" by short story author Ethel Lyne White had Nurse Cherry (Crosson in the TV episode) realize Nurse Ames was Edward Leeks in disguise in the climax. And as he struggles to close the window which is rattling from a storm wind, (and still doing his phony drag nurse shtick), Nurse Crosson/Cherry pushes him out the window to his death. Teleplay scripts changed this to Nurse Ames revealing herself to be Edward Leeks on purpose, and then killing Nurse Crosson in a chilling ending. Hitchcock himself suggested the detail of the nurse struggling with the killer and ripping open his shirt; revealing his hairy chest for the audience for added terror.
