At the end of the movie it's revealed that Tony has been telling this whole story to Bruce Banner. Banner tells Tony he's not that kind of doctor, a fact Tony well knows, because in Avengers Tony informed Banner that he's a fan of his work. Banner also tells Tony that he doesn't have the "temperment" to be a therapist, an allusion to his angry alter ego The Hulk.
