Esther Mullins is shown as bedridden for twelve years following the death of her daughter and seemingly no explanation is given as to why she is suffering. However, if one takes the story of the other Conjuring prequel, The Nun: La vocazione del male (2018), into context, that demonic presences can infect and distort both land and people physically, the presence of Annabelle (the demon using the doll as a totem) has infected Esther. That, plus the demonic attack upon her originally by the Demon, which is revealed to have graphically disfigured her and forced her to wear a mask to hide her scarring, shows that there is a good theory as to why Esther Mullins is bed-ridden and hurt.
