Director Neil Marshall didn't have one specific ending in mind during filming, so he agreed to make an American version where Sarah escapes from the cave. This was considered by many to be the "happy ending" (in contrast to the "unhappy" UK version where Sarah doesn't escape), but Marshall likened it to the finale of Non aprite quella porta (1974), stating that "Just because [someone] gets away, does that make it a happy ending?"
