Many of the crimes featured in Cold Case are based on real-life cases. These include the murder of Emmett Till, the Columbine school shooting, and, most notably, Philadelphia's real-life "Boy in the Box" case, which as of the year 2021 still remains unsolved. Back when Cold Case aired its fictional version of the "Boy in the Box" case in 2003, it regenerated some public attention to the story, and similar crime drama TV shows did their own versions of the story. Cold Case was the only popular crime drama series at the time to feature a plot where psychiatric abuse and a private orphanage scandal were the causes of death; other shows, like Law & Order: Special Victims Unit and CSI: Crime Scene Investigation, featured plots where systematic child abuse was the cause of death. In the real-life case, it is generally the accepted theory that the murder victim died due to some form of child abuse, as the unknown boy was severely malnourished and had scars on his body.
