It is never explained how the man in the coffin got out with the baggage claim door locked and the attendant sitting just feet from him, but apparently not seeing him. Although it is not explained, as pointed out, from Holmes' comment (that the Attendant is passing the time by using Mathematics to relax) in the inspection of the Baggage Car, coupled with Attendant's Murder, it is logical to infer the Conclusion the Attendant is A Henchmen/Gang Member of Colonel Sebastian Moran/Major Duncan Bleed, who is used to let out and let in the Murdering Henchmen, who hides in the Coffin until needed. Finally, the Attendant is killed when he has outlived his Henchman use, by being suspected of being in the Gang by Sherlock Holmes, who'd intended to Interrogate him again.
