When Captain Kirk fights Martia in her Kirk form, this was easy for the Klingons to know which Kirk to shoot because Martia had no leg irons which you can see just before the two Kirks roll over Dr. McCoy. Additionally, when Martia-as-Kirk says, "Shoot him," and points to the real Kirk, she points way too high, indicating she was not used to being this height. The real Kirk points back at the correct height.
