The scene on Mt. Rushmore where Eva Marie Saint slips down and bashes her elbow actually happened. Both the pain she shows and Grant's reaction, are real. By contrast, the scene where she is dangling from a cliff was an illusion. She was laying on a cliff prop that was sloping 45 degrees, so in no danger of falling.
