In the hospital demolition scene, Joker comes out from the hospital and he actives the detonator to make it explode, but the explosion stops suddenly and Joker turns around making a wild gesticulation with the arms, confused, playing with the detonator until that it explodes again, scaring him and getting onto the bus to escape. A modern urban legend says that this was unplanned, but in reality, as Christopher Nolan explains in the special feature Gotham Uncovered: Creation of a Scene (2008) included in the film's home release, special effects supervisor Chris Corbould devised the moment as a way to get Heath Ledger out of the building and then, in Nolan's words, "literally drop the building to the ground" without harming the actor, all in one shot.
