The original spec screenplay by screenwriter David Loughery was much more of a throwback to 90's erotic thrillers but the director Douglas Aarniokoski performed several rewrites. Whilst most of what you see in the finished film is what Loughery wrote, there are several key changes from script-to-screen: 1. In the original script, Steve was a chef. In the movie, he works as as paramedic at the same hospital as Danni. Their original script sex scene also took place in a hospital pool at night and not an unoccupied room of the hospital during work hours. 2. In the original script there was no narration from Abby during any of the scenes. 3. In the original script there was no brief romance between Danni and Abby; they are merely co-workers and friends. 4. Kathleen Turner's scene with all the nurses near the beginning of the film was not in the original screenplay. 5. In the original script's ending, Abby seduces Steve after being invited to dinner. Danni finds them in bed and kills Steve mid-sex with Abby's hair pin, revealing that she had been testing Steve all along. The following final scene is of Abby changing her identity to Rachel and starting a new job, almost exactly how it plays out in the movie. 6. There was no hospital rampage from Abby in the original script. This was more than likely a request from producers/Lionsgate to add more blood and gore to the movie.
