This picture examines the strong bond formed between Dr. Libbie Bowen (Lena Olin) and her patient, Mr. Jones (Richard Gere), and the psychiatrist's painful realization that her human feelings and ultimately her actions have violated her ethical standards. The illness that first brings Mr. Jones into the doctor's life is a severe form of mood disorder. The movie's production notes stated: ''Some twenty million Americans suffer from mood disorders, and of these a conservatively estimated two to three million are afflicted with various forms and degrees of manic depression, also known as bipolar illness.''
