When a police officer tells Detective Hoffman "another doctor went missing from the hospital", this is a clue that the events in this film occur before John Kramer's death in "Saw III" (2006). When John died, Dr. Lynn Denlon, who was abducted from the hospital, also died in the same room as John. Therefore, her corpse would have been discovered and she wouldn't be labelled as "missing".
