As explained in the novel, Larry Sportello's 'Doc' nickname comes from his early skip-tracing days. Being reluctant to carry a weapon, Doc is instead given a hypodermic needle containing sodium pentathol (AKA: 'truth serum), that he uses to persuade potentially problematic people.
