The Dark Curse is cast a total of five times throughout the series: once by Regina (who sacrificed her father), once by Peter Pan (who sacrificed Felix, his right-hand man), once by Snow White (who sacrificed David, although he was resurrected shortly thereafter), once by Hook as dark one in Camelot, sacrificing Merlin (as Nimue, the first dark one), and finally by Regina, who is tricked by Ivy into using her own blood, as she has already killed the one she loved most.
