While everyone who definitively gets killed by Arthur Fleck in this movie was legitimately bad and unkind to him, the only victims of his who were directly threatening him were 2 of the 3 Wall Street traders, whom Arthur shot and killed while they were beating him up. The other people who were murdered--the 3rd trader, Penny Fleck, Randall, and Murray Franklin--were not threatening Arthur physically when he murdered them, though it could be argued that all of them but Penny were threats to Arthur in another way (the 3rd trader as someone who could identify Arthur as the killer of his 2 trader buddies, Randall because he correctly figures out that Arthur did something bad with the gun Randall gifted to him, and Murray because he threatens to humiliate and ostracize Murray in public and send him back into obscurity). If Arthur did in fact kill the Arkham Asylum psychiatrist in the movie's final scene, that would be similar to his killing of Penny in that neither woman was any threat at all to him; the fact that she does nothing wrong or negative towards him is a reason that her murder, if it happened, would be a complete outlier in the film.
