This originally began development in 2016 at Showtime, under the title "The City". It went into turnaround ( Hollywood lingo, which refers to the status of a project when a studio decides not to develop it further and offers it to other studios to recoup development costs) and the next year, Fox bought the series from Showtime. The original concept dealt with a new resident arriving at a hospital and gradually learning that his supervisor, a senior resident, is a murderous psychopath. This was discarded once the series moved to Fox.
