In the revived Doctor Who (2005) Missy (Michelle Gomez) whom is the first female Master claims that she knew The Doctor when he was a little girl. The First Doctor (William Hartnell) was established as The Doctor's first incarnation and his Regeneration into Patrick Troughton was the first time The Doctor regenerated. If so, this would mean The First Doctor is really the Second Doctor and that his first on-screen regeneration was really his second. The Doctor eventually becomes a woman in the 11th season of the revived series and is played by Jodie Whittaker.
