Even though it is very debatable whenever old Rose was asleep or dead at the end of the film Rose in her youth is seen 'reunited' with Jack in the grand staircase surrounded by those who died in the sinking including Fabrizo, Tommy Ryan, the ship's band among others. Judging by this it is strongly implied Rose did die of old age as foreshadowed by Jack before he died and the pictures of the things Rose did in her youth suggest she lived her life as Jack said she would. This could also suggest old Rose knew she was ailing and made it her dying wish to rid of the 'Heart of the Ocean' by throwing it into the ocean.
