At Clint's home when Tony talks to Fury in the barn and tells him about the nightmare that Wanda showed him in the beginning; Fury realizes that not only is Tony's worst fear seeing everyone dead but that he survives and has to live with his guilt about being the only one left. This comes partially true in Avengers: Infinity War (2018), where Thanos succeeds and wipes out half of all life in the universe. In Avengers: Endgame (2019), however, Tony sacrifices his life so that he dies and everyone else lives.
