During the scene where Helen Sharp gets evicted from her apartment by a group of police officers while watching a film starring Madeline Ashton, she keeps rewinding the scene where Madeline's character is strangled to death. This foreshadows when Madeline is almost strangled to death by Ernest and when she dies of her neck being broken. During the scene where Ernest confronts Madeline about her mistreatment of him, she insults him and his impotence. In anger, this causes him to strangle her over the stairs until he stops once he briefly came to his senses. Then, when she insults him again, he pushes her down the stairs, which caused her to fatally break and twist her neck.
