In the concluding scene, it's revealed that Don swapped places with Vijay and induced heavy dozes of anaesthetic in his IV leading to cardiac arrest and eventually death. Dr. Khilwani and his team would've seen through the folly as Don wasn't even in coma when handed over by Vardhaan and sedatives Don was subject to in order to keep him alive couldn't have led to cardiac arrest of such scale.
