When Agent Dan Erickson and Agent Lindsey Perez go to the audio lab with Detective Mark Hoffman they already suspect him of being Jigsaw's accomplice, the analysis of the audio tape just confirms it. Yet they go there with him alone, they don't bring any backup at all. This goes against FBI procedure; they suspected a man who is extremely dangerous and a trained, experienced killer and they should have known that once it was revealed that Hoffman was indeed Jigsaw's accomplice they would be dealing with a man who had nothing left to lose and there is no one more dangerous than a man in Hoffman's situation. Standard procedure when dealing with a potential suspect as dangerous as Hoffman would have been to have a FBI Tactical Team on standby outside the room and signal them when their suspicious were confirmed.
