After the attack on the top brass meeting, Scotty, when explaining how Khan escaped, refers to Kirk as 'Captain'; however, Kirk had been demoted to First Officer, so should have been addressed as Commander. In addition, when walking into the top brass meeting, Kirk has four insignia on his shoulders representing his being a Captain when he should have only three as a Commander, as Spock does.
