When Makkari is in ancient Mesopotamia using sign language there are several signs that in those times would have made no sense to the population. Like the word "time" where she points at her wrist, they would not know what a wristwatch is so that would not have been the sign for time. And since the Eternals are not time travelers, Makkari herself wouldn't even have known what a wristwatch is at that time, so her sign language couldn't include that specific sign.
