This was changed on the DVD, but on the original version of the movie, during the battle of Helms Deep the orcs launched ropes with hooks on them at the top of the wall. They started to hoist the big ladders with the ropes. Two scenes later they pushed the ladders the rest of the way to the wall but the ropes were not there.
