When B.A. is extracted from the prison bus, they fire a rocket-powered anchor/grappling hook into the emergency exit of the bus. It burst through the door and then B.A. jumps on to the door as it's towed by the pickup truck. But where he lands on the door is exactly where the anchor was protruding through the door in the previous scene and B.A. would have been impaled on the anchor.
