At the beginning of the film, the scar on Alice's left shoulder is an older, healed scar. At the end of the film, when she wakes up on the exam table, the scar is puffy and pink like new scar tissue would be. The new wound on the old scar line wouldn't cause the whole scar to look pink and inflamed.
