Although (as is learned later) Baymax removes Tadashi's care card and places it in his fist when he sacrifices himself for Hiro and Abigail, he is still functioning as if he has it in him right up to the end, for example by asking whether Hiro is satisfied with his care. However, there's a pause right before the rocket fist fires, when Baymax is off camera and Hiro looks away, a moment designed to cover the literal hand-off.
