Doctors do not ride in ambulances because ambulance services are overseen by their own physician. A paramedic would have been assigned. Moreover, with the driver inside to relieve himself, the medic riding with a patient cannot legally leave the patient alone in the ambulance or that would be abandonment. Finally, the patient has Swamp Fever. While that disease exists, it is caused by a virus that is often fatal to horses but does not affect humans at all.
