The Doctor is basically a computer program, albeit a sophisticated one that learns and adapts and exhibits "personality". However, he is still a program. His mobile emitter (introduced in season 3) allows his entire program to be downloaded into it and used away from the ship. However, this would essentially create a copy of the Doctor (like we would copy a program onto a flashdrive), so there is no reason why the Doctor cannot always be present in Voyager's sick bay regardless of whether he was on an away mission.
