In the Official Illustrated Screenplay and Peter David's novelization, the Hulk covers Betty's mouth with his hand before the Hulk-dogs appear. But the Hulk's hands are big enough to wrap around someone's head or body; he can easily cover a person's mouth with one or two fingers.
