The day after the Guardian Angel first appears, Susan divulges to the psychiatrist that she has seen what she thinks is a guardian angel. But at this point the angel did not tell her who he was; in fact, he had not even spoken. And she had not mentioned a guardian angel to Lorenzo when she discussed her fear the night before. The story doesn't explain why she came to believe that what she saw was a guardian angel.
