As Indy pushes the block out of the wall to escape from the crypt (after the snake encounter in the Well of Souls), there is a close up, from the outside, of the block being pushed out. In the next frame, as Indy and Marion crawl out, you can see a crew member crouching down (presumably the cameraman) from the perspective of the previous scene.
