Early in the film, when Sarah wakes in the hospital, she exits her room to a completely abandoned corridor. Even assuming she is imagining the lack of people (as she is presumably imagines the lights going off in succession), in reality there would be a constant flow, and at least one person would stop her from running. Also, when she runs into Beth and other people in the hospital "appear," visitors, doctors, nurses, and interns all walk past her frantic, sobbing, limp figure without reacting at all. Of course, it's possible that the audience is seeing this entire scene from her point of view, which might be selective in what she experiences.
