(at around 1h 15 mins) Stark accidentally detonates an ammo pellet taken from one of the Red Skull's weapons while examining it in a safety chamber. Although the observation window explodes in Stark's face with enough force to blow him across the room, the only injury he suffers is a small cut over his right eyebrow.
