At the conclusion of the film when Costa stabs Ileana in the stomach because he thinks she is pregnant with his child, he doesn't notice or register that there is no blood or fluid coming out of the location where the scissors were thrust into her "pregnant stomach". Something like that would have been extremely obvious to a calculating psychopath like Costa when he was looking right at Ileana and her stomach after stabbing her, and Ileana should not have had that much time to fake being injured before she ripped the scissors out of the prosthetic bump and fatally stabbed Costa in turn.
