When Paul Kersey forges a doctor's ID, he drives passed the security checkpoint, to the hospital where Charles Wilson "Nirvana" is held, where we clearly see a STOP sign and a guard, but Paul Kersey doesn't stop and the guard pretends not to notice this obvious security breach. The hospital that holds violent criminals cannot let any car through without authorization. After an ID theft of a staff doctor, the guard would surely have this doctor's name on his list. So even with a fake ID, in real world, Paul would never had been able to access even the hospital parking lot, let alone the guarded building.
