Anton Phibes, un celebre organista dato per morto in un incidente stradale, in cui è rimasto soltanto orribilmente sfigurato, è convinto che la sua bella moglie Victoria Regina, perita durante un'operazione chirurgica, sia stata vittima dell'incapacità dei medici. A distanza di anni dal fatto, Phibes, che amava profondamente la moglie, comincia ad attuare la sua vendetta contro gli otto chirurghi e l'infermiera che avevano operato la consorte. Ispirandosi alla Bibbia (le dieci piaghe che colpirono l'Egitto in seguito alla maledizione divina) e aiutato da un'avvenente e silenziosa collaboratrice, Vulnavia, Phibes uccide, l'uno dopo l'altro, e coi mezzi più atroci, sette medici e l'infermiera.
