Bree è un transessuale. Alla vigilia dell'intervento chirurgico che la ridefinirà sessualmente, scopre di avere un figlio, Toby, nato vent'anni prima dal suo unico rapporto eterosessuale. La psicoterapeuta, che prepara Bree a "passare" alla sua nuova condizione sessuale, la costringe a confrontarsi con il ragazzo e con il passato. In caso contrario le negherà l'autorizzazione legale a procedere con l'operazione. Toby, intanto, arrestato per droga e prostituzione, ha bisogno di un padre. Abusato dal patrigno e precocemente orfano di madre, il ragazzo conduce una vita dissipata e promiscua. Sarà Bree a pagare la cauzione e il riscatto per una vita migliore, conducendo Toby in un viaggio di formazione attraverso l'America.
