This movie was originally intended to have a sequel to form a trilogy, as opposed to a duology. However, due to this movie's box-office thunder being taken away by the release of Il cavaliere oscuro (2008) on the second week of its premiere, this movie's attendance dropped. Despite making more money than its predecessor, the profit was deemed disappointing, and studio's interest in a third movie waned. Guillermo del Toro still expressed interest in directing the sequel, but had to put it on hold due to commitments to other movie projects. In 2012, Ron Perlman donned his Hellboy costume to meet a terminally ill boy as part of the Make-a-Wish foundation, which inspired del Toro to do everything he could to get the sequel started. This led to a five-year Development Hell, where no producer nor studio expressed interest in funding the proposed sequel due to its prohibitive costs (the necessary budget was rumored to be $120 million). At one point, del Toro wanted to publish the screenplay as a comic instead of a movie, only to get vetoed by Hellboy's creator Mike Mignola (who wanted exclusive creative control over the comics). Inspired by the success of the green-lighting of Deadpool (2016), Ron Perlman tried to convince fans to start trending #HellboyIII on their Twitter accounts to help showcase that there is an audience for Hellboy. In 2017, del Toro did a similar thing on his Twitter account by posting a poll for his followers: if 100,000 votes were cast, del Toro would have a sit-down meeting with Perlman and Mignola on the development of the proposed sequel. After exactly 132,938 votes were cast, the meeting happened. However, afterwards, del Toro made the announcement that the sequel was "100% not happening", because Mignola decided to support re-working the existing Hellboy III script into the unrelated reboot Hellboy (2019) (by specifically "stripping out the del Toro-isms" and making it much more violent). Thus, Hellboy II became the final chapter as opposed to the middle. However, after the critical and commercial failure of the reboot, Ron Perlman stated that the third film could still be made if they find a studio interested on it.
