All'interno del Dipartimento per la Ricerca sul Paranormale e la Difesa c'è aria di maretta. Il muscoloso detective Hellboy, per gli amici Red, è vittima tanto del suo capo Manning, che vorrebbe impedirgli di mettersi in mostra davanti agli umani, tanto della sua pirotecnica fidanzata Liz, che sembra avere un segreto e non volerglielo rivelare. Ma un pericolo molto più grande incombe sul B.P.R.D. e sul mondo intero: il principe Nuada del mondo delle tenebre, stanco di obbedire da secoli all'umanità, ha risvegliato un esercito leggendario di macchine assassine al fine di annientarla. A Hellboy e al suo indistruttibile pugno di pietra il compito di fermarlo.
