Il misterioso fenomeno che vede i morti tornare in vita e procacciarsi carne umana, si è ormai esteso anche al di fuori dei margini continentali. A Slaughter Island, a largo delle coste della Pennsylvania, i clan rivali degli O'Flynn e dei Muldoon si contendono il dominio dell'isola e la strategia da adottare contro i morti viventi. A trovarsi in minoranza è il vecchio O'Flynn, sostenitore di un'uccisione rapida e totale dei non-morti, che viene condannato dalla famiglia nemica al confino sulla terraferma. Nei pressi del porto, O'Flynn si unisce a un gruppo di militari ribelli intenzionati a raggiungere l'isola e li convince a passare dalla sua parte nella battaglia contro gli zombi e per il dominio del territorio.
