Ivan Drago, campione sovietico dei pesi massimi categoria dilettanti, arriva negli Stati Uniti per una esibizione fra lui e Rocky Balboa, che ormai ritiratosi dal ring rinuncia. Si fa avanti allora Apollo Creed, ex campione del mondo ritiratosi da ormai cinque anni, che perde però la vita durante l'incontro. Spinto dal desiderio di vendetta e rivincita Rocky accetta di combattere a Mosca il giorno di Natale.
