Nel 1954, i due agenti federali Teddy Daniels e Chuck Aule vengono inviati con un battello a Shutter Island, a largo della costa est, per investigare sull'improvvisa scomparsa di una pericolosa infanticida residente presso l'istituto mentale Ashecliffe, Rachel Solando. Il direttore dell'istituto, il dottor Cawley, e i vari infermieri sostengono che la madre assassina si sia come dileguata dalla sua stanza senza lasciare alcuna traccia, ma l'agente Daniels pare nutrire fin dal principio dei forti sospetti sul modo di condurre l'ospedale da parte del dottor Cawley e del suo medico assistente, il dottor Naehring. Un uragano costringe i due agenti a protrarre il soggiorno sull'isola, durante il quale le indagini proseguono e particolari sempre più inquietanti emergono, mentre Daniels continua ad avere delle visioni che riguardano la moglie defunta e le sue esperienze di guerra contro gli ufficiali nazisti.
