Il boss Anjo viene ammazzato da un misterioso killer in maniera estremamente truculenta, ma nessuna traccia viene lasciata. Per ritrovare Anjo, convinto che sia ancora vivo, il folle e sadico Kakihara, suo vice, accetta il consiglio dell'enigmatico informatore Jijii e per questo rapisce e tortura Suzuki, capo di un banda rivale, che si dimostra però estraneo all'accaduto. Per punizione Kakihara e il suo clan vengono estradati dal sindacato della yakuza di Shinjuku. La caccia non finisce, ed è sempre più complessa per Kakihara che non può immaginare che il killer sia Ichi ("uno"), un ragazzetto timido e complessato che, sotto il condizionamento mentale di un misterioso manovratore, si trasforma in una brutale macchina di morte. Ossessionato dall'idea di confrontarsi con Ichi, Kakihara tortura chiunque dimostri di saper qualcosa sul suo conto mentre il suo rivale continua di nascosto a seminare morte.
