Dopo aver ucciso la madre e il suo amante alcuni anni prima, Patrick è il paziente comatoso della camera 15 in una remota clinica psichiatrica privata gestita dal dottor Roget, che lo usa come cavia nei suoi studi bizzarri sulla vita e la morte. Kathy, un'infermiera che si è da poco separata dal fidanzato, inizia a lavorare presso la clinica e viene incaricata di prendersi cura di lui. Si rende presto conto che l'assassino è in grado di comunicare, e grazie alle sue capacità psicocinetiche, cerca di manipolare gli eventi nella vita di lei. Man mano che la comunicazione di Patrick diventa più forte, strani e terrificanti eventi cominciano a verificarsi e i sentimenti che l'assassino nutre per Kathy si stanno per manifestarsi come una mortale, sanguinosa ossessione.
